Heart coherence is a slow breathing technique — approximately 6 cycles per minute — that acts on the autonomic nervous system by amplifying heart rate variability. Studied in over 200 scientific publications and recommended by the French Federation of Cardiology since 2014, it is accessible to everyone, without any equipment. Here is what the science actually says.
What is heart coherence?
Heart coherence is a slow, rhythmic breathing technique designed to synchronize the heart’s rhythm with the breath. Its principle rests on a well-documented physiological phenomenon: heart rate variability (HRV).
Contrary to what one might expect, a healthy heart does not beat at a perfectly steady pace. Slight variations occur between each beat — and this is a good sign. These variations reflect the autonomic nervous system’s ability to adapt to its environment: effort, rest, stress, relaxation.
When we breathe, our heart rate naturally follows: it accelerates slightly on the inhale and slows on the exhale. This phenomenon, known in medical terminology as respiratory sinus arrhythmia (RSA), is at the core of heart coherence. By breathing at a slow, regular pace — approximately 6 breaths per minute — these variations are amplified harmoniously, activating the parasympathetic nervous system (the body’s recovery and calming system).
Where does this practice come from?
The effects of slow breathing on body and mind have been known for millennia, notably through pranayama techniques in yoga. But the modern approach to heart coherence emerged in the 1990s, with the work of the HeartMath Institute in California, which studied the relationship between heart rhythms, emotions, and cognitive performance.
In France, it was Dr David Servan-Schreiber who introduced heart coherence to the general public in 2003, in his book Healing Without Freud or Prozac. Subsequently, Dr David O’Hare popularized a practical method that has since become a reference: the 365 method — 3 times a day, 6 breaths per minute, for 5 minutes.
Since 2014, the French Federation of Cardiology has cited heart coherence as the first of its 10 golden rules for reducing stress, as part of cardiovascular disease prevention.
How does it work, concretely?
The principle is simple. By voluntarily slowing one’s breathing to approximately 6 cycles per minute (5 seconds inhale and 5 seconds exhale), one stimulates the vagus nerve — the main nerve of the parasympathetic nervous system, connecting the brain to the heart and digestive organs.
This stimulation produces several measurable physiological effects:
- Increased heart rate variability: the heart recovers a broad, regular oscillation, a sign of good autonomic nervous system balance.
- Activation of the parasympathetic system: the body shifts from an “alert” mode (sympathetic) to a “recovery” mode (parasympathetic).
- Reduction of cortisol: the stress hormone decreases, producing cascading effects on blood pressure, digestion, and sleep quality.
This is not relaxation in the passive sense. It is an active training of the nervous system, somewhat like training a muscle. The more regular the practice, the faster the body learns to return to this state of balance.
What the science says
Research on voluntary slow breathing and heart rate variability is a growing field. Here is an overview of the main available data, presented with the nuance they deserve.
Encouraging meta-analyses
A 2022 meta-analysis published in Neuroscience & Biobehavioral Reviews (Laborde et al.) reviewed 223 studies on voluntary slow breathing. Its findings show a significant increase in parasympathetic-linked heart rate variability (measured by the RMSSD indicator) at three moments: during the exercise, immediately after, and after several weeks of regular practice. The authors conclude that slow breathing represents an accessible, low-cost complementary intervention.
A second 2024 meta-analysis published in Mindfulness (31 studies, 1,133 participants) confirmed that slow rhythmic breathing produces a significant reduction in systolic blood pressure and a moderate increase in heart rate variability indicators.
Observed effects on stress and anxiety
Several studies have observed that breathing at 6 cycles per minute, with or without biofeedback, is associated with reduced symptoms of panic, anxiety, negative emotions, and chronic pain, as well as improved quality of life and stress resilience.
A 2022 randomized controlled study (Psychophysiology, Laborde et al.) involving 112 participants showed that a 5-minute session of slow breathing at 6 cycles per minute produces comparable physiological effects whether or not a biofeedback device is used. In other words, a simple visual breathing guide is sufficient.
A clinical trial conducted with young adults (Cureus, Chaitanya et al., 2022) showed that after 4 weeks of daily resonance frequency breathing practice (6 to 6.5 breaths/min), participants demonstrated significant improvement in heart rate variability, reduced perceived stress, and better cognitive performance.
INSERM’s position
In 2023, INSERM (France’s National Institute of Health and Medical Research) published a detailed analysis in its Canal Détox series, titled “Cardiac coherence — a technique to improve your health, really?” Its position is nuanced and instructive:
- Slow breathing exercises appear beneficial for stress and anxiety.
- The research field is “increasingly fertile” — nearly 2,000 clinical trials studying heart rate variability are registered on clinicaltrials.gov.
- However, many existing studies have methodological limitations (small sample sizes, lack of control groups, heterogeneous protocols).
- heart coherence should not replace treatments whose benefit is well established by clinical research.
- The exercises are likely free of risk and cost only the time devoted to them.
This is a balanced position we share: heart coherence is a promising complementary tool, not a miracle cure.
The 365 method in practice
The most widespread method is simple to remember:
- 3 times a day
- 6 breaths per minute
- 5 minutes per session
This means inhaling for 5 seconds, then exhaling for 5 seconds, continuously for 5 minutes. The practice is done seated, back straight, in a relatively quiet place.
Common variations
Depending on needs, other breathing rhythms are used:
- Balanced rhythm (5-5): 5 seconds inhale, 5 seconds exhale. This is the classic heart coherence rhythm, suitable for most situations.
- Relaxing rhythm (4-6): a longer exhale than inhale. This rhythm accentuates parasympathetic activation and promotes deep relaxation.
- Box breathing (4-4-4-4): inhale, hold, exhale, hold — 4 equal counts. Used in particular for concentration and acute stress management (fighter pilots, special forces).
The important thing is not to find the “perfect” rhythm, but to practice regularly. Benefits develop gradually, generally after 2 to 4 weeks of daily practice.
Who is heart coherence for?
heart coherence is accessible to everyone, with no prerequisites or equipment. It is used in varied contexts:
- Daily stress management: before a meeting, an exam, a moment of tension.
- Sleep improvement: a session before bed helps activate the recovery system.
- heart rehabilitation: integrated since the 1990s in certain hospital programs.
- Performance and concentration: used by French Air Force fighter pilots, high-level athletes, and professionals in demanding situations.
- Therapeutic support: as a complement to psychotherapy, sophrology, or meditation.
What heart coherence is not
To be honest about what this practice can — and cannot — do, it is important to clarify:
- heart coherence is not a medical treatment. It does not cure anxiety, insomnia, or hypertension.
- It does not replace medical or psychological care when such care is necessary.
- Benefits are gradual and linked to regularity: an isolated session provides momentary relief, but lasting effects require daily practice.
- It is complementary: it integrates well into a wellness routine alongside physical activity, a balanced diet, and appropriate medical follow-up.
